A peek behind the prescribing curtain: The top challenges facing prescription-writer solutions

In many of my blogs, and, frankly, in many of the blogs written by my colleagues here at Altera, one theme is inescapable: Evolution. Or, maybe a better way to put it is that there is an underlying driver for everything we do. Evolution. That’s a loaded word, often, but in the health IT world, it’s what keeps us focused on the summit of healthcare. It fuels us to be better today than yesterday.

I want to focus on one aspect of health IT that is facing challenges that need to be addressed: Electronic prescription-writing solutions.

For years, electronic prescription writers (eRx) have been reshaping how healthcare providers manage and transmit prescriptions. Here’s a look at the most pressing issues prescription writer solutions must overcome so that their benefit continues growing for providers and patients.

Workflow and usability issues

To bring real value to the people using these eRx solutions, the technology needs to work for the users—not against them. The concept of usability should be simple enough, but often with prescribing solutions, clinicians can face disruptions in the workflow that lead to frustrations and can result in medication errors. Errors when canceling or renewing prescriptions (e.g., “bad_prescription_id” or missing prescriber order numbers) slow down the workflow and can lead to incomplete or inaccurate prescription data flowing into downstream systems. User interface issues, such as missing options or confusing layouts, also cause problems when prescribing critical medication to patients who need it most. Usability for these solutions is key.

The concept of usability should be simple enough, but often with prescribing solutions, clinicians can face disruptions in the workflow that lead to frustrations and can result in medication errors.

Integration and interoperability hurdles

As a practicing nurse, I can attest to the fact that there’s a lot of data swirling around any healthcare organization. From multitudes of patient data, to a hostful of other clinical information, eRx solutions shouldn’t make clinicians’ jobs more difficult. They must integrate seamlessly with EHRs, pharmacy benefit managers and third-party vendors. But it isn’t always so easy. Challenges on this front often include mapping medication data accurately between systems, handling workflow variations across care settings and ensuring proxy agents and various provider roles have appropriate access. Better integration within the EHR will support cleaner processes and better overall usability, while, of course, helping keep patients safe on their care journeys.

Navigating a rapidly changing regulatory landscape

The regulatory environment for e-prescribing is more complex than ever. And, perhaps more than ever before, organizations should be able to rely on eRx solutions that can meet these regulatory needs. New federal requirements are rolling out, including:

  • NCPDP SCRIPT Standard Version 2023011 (required by Jan. 1, 2028)
  • NCPDP Formulary & Benefit Standard Version 60 (by Jan. 1, 2027)
  • NCPDP Real-Time Prescription Benefit (RTPB) Standard Version 13 (by Jan. 1, 2027)

However, challenges persist in accurately tracking and reporting on prescription activity and supporting new reporting requirements for real-time prescription benefits and electronic prior authorization. Healthcare organizations rely on eRx systems to support quality reporting and regulatory compliance.

The goal of these standards is to improve interoperability, transparency and patient safety, but they require significant updates to existing systems and workflows. This is when it is crucial for healthcare organizations to partner with a comprehensive health IT vendor to ensure all aspects of the eRx solution are compliant and working as it ought to.
